Error NP-34981-5 on the PlayStation 4 began to appear on the forums more than six months ago, but to this day there is practically no information about it. Have patience

Remove trophies
Most of the user notes that they managed to fix the crash through cleaning the partition “Trophies”… It is worth deleting games that do not have at least one achievement. It is not clear why this method works, but the fact remains. It is speculated that deleting these games causes the console to re-sync the games library. During synchronization, the error is defeated.
What do we have to do:
- Log in to the PSN profile where the problem appears.
- Go to the “Trophies” page in the toolbar.
- From the entire list of games, you should find the one in which the trophy is missing. If the game already has an earned trophy, it cannot be deleted.
- Go to the “Options” of the found game and select the “Delete” option from the menu on the right.
- Try to start the game.
If everything went well, this time the error should not appear.
System initialization
Upon completion of the procedure, all system settings are reset to their original state. At the same time, all saved data are deleted from the memory of the console: games, files, user data. Initialization does not affect data on external drives connected via USB. It is better to download all useful data to a USB flash drive or external HDD in advance.
How to start initialization:
- Go to the “Settings” menu.
- Select the “Initialization” item.
- Click on the “Initialize PS4 System” button.
- Confirm your intention several times and wait for the completion of the procedure.
- Reinstall the game and open it.
